​​​​​​​​​​​​​Middlesbrough Children Matter has permanent job vacancies for Outreach Practitioners to work 32.5 hours per week based in Middlesbrough offering a salary between £22,183 - £23,541 pro rata.

We are looking to appoint Outreach Practitioner's to support inclusion and build inclusive capacity in settings. You will be contributing to the assessment, planningand delivery of high quality interventions for children and young people with SEND or those at risk of permanent exclusion. Also you will work alongside children and young people, families, staff in settings and other agencies. As post holder you will be supporting the delivery of a 0-25 Outreach and Inclusion model and supporting the timely and effective identification and assessment of children and young people who require additional support.

Duties & Responsibilities:

  • To signpost and provide support, information and advice to a range of stakeholders including but not limited to settings, parents and other agencies.

  • To have a good understanding of either early years, primary, secondary or post 16 education/ curriculum and contribute to the planning for individual and groups of children and young people.

  • To contribute to the development and implementation of individual education and/ or SEN Support Plans, intervention, care and therapy programmes for children and young people.

  • To provide appropriate educational support and have regard to the health needs of all children, maintaining a safe environment including all safeguarding considerations.

  • To provide specialist support for children who require specific or individualised therapy programmes to promote mobility, correct positioning, and the use of the appropriate equipment which will facilitate wellbeing, learning and development – for example, specialist seating/standing frames/other mobility aids.

Requirements of the Post include:

1. A recognized qualification at Level 3 in an appropriate area (e.g. child develop/ SEND).
2. English and Mathematics at GCSE grade C or above.
3. Knowledge and understanding of education and curriculum in either early years, primary, secondary and post 16.
4.Sound knowledge of child and young person’s development.
5. A positive and effective approach to behavior management.
6. Proven exemplary practice in either early years, primary, secondary or post 16.
